Instructions for completing the on-line application

If you have not completed the eligibility tool – we strongly recommend you return to the Eligibility Tool for determination of your eligibility.

This process will require 30-45 minutes of time to enter your information. Your application will be saved when you are assigned a claim number on the last page of the on-line application tool. All data entered will be lost if you fail to complete the entire application.

What You Will Need
Please be prepared to send the following documents with your application. These documents will also assist as you in answering questions in the on-line application tool:

1. Active Duty Service Members – military documents from on-line, or letter from commanding officer or personnel director, showing dates served in-theater or non-theater.
2. Veteran – DD-214 (member copy 2 or 4) for the period(s) when you were deployed in-theater or non-theater for which you are applying.
3. Proof of current Ohio residency.
4. Proof of residency in Ohio at time of entry in the U.S. armed forces.
5. Veteran Injured Retired/Disabled – DD-214 (member copy 2 or 4) and military records or VA rating showing combat related injury.
6. Family Member – please provide a death certificate and a DD-214 of veteran; or DD-1300, and documents which apply to you:
   1. Spouse: marriage certificate to veteran.
   2. Child: child(ren) birth certificate or adoption records identifying veteran as parent, and if applicable death certificate of deceased parent.
   3. Parent: veteran's birth certificate or adoption records, and if applicable death certificate of deceased parent.
7. Guardian – Veteran's DD-214 (member copy 2 or 4) and legal guardianship documents (if veteran is deceased provide copy of death certificate).
