If you do not know your Claim Number, please call the Ohio Veterans Bonus Program at 1-877-OHIO VET (1-877-644-6838) for your claim number or to check the status of your application.

We strongly encourage using the on-line application tool as it reduces application processing time. However, if you wish to submit a handwritten application form, Veterans and Current Service Members can click here to print a copy of the form. Family members applying for the bonus can click here.
